Immunization Requirements
Salem-Keizer School Public Schools is required by Oregon state law to keep a record of each student’s immunizations. When enrolling a student, please complete a Certificate of Immunization Status with your child's record of immunizations.
Kindergarten students must have a minimum of one dose of each required vaccine before enrolling in school.

Immunization Requirements by Age
At all ages and grades, the number of doses required varies by a child’s age and how long ago they were vaccinated. Other vaccines may be recommended and exemptions are also available. Please check with your child’s school, child care or healthcare provider for details.
Where Can I Get Immunized?
Immunizations are available at your student’s doctor’s office, at many pharmacies, or at county health departments. Call for information or to schedule an appointment.

Immunization Reports
Our immunization reports include graphs in both English and Spanish showing how many school-age kids are up to date on immunizations at our locations.
